Service overview

EchoMed provides AI-assisted drafting of clinical notes based on clinical content you provide without patient identifiers.

Clinician responsibility

You are responsible for reviewing, editing, and approving all notes before use in any clinical system.

No patient identifiers

Do not include patient identifiers or other protected health information. You are responsible for ensuring all inputs are free of patient identifiers before submitting.

Compliance acknowledgments

Before using dictation and note generation features, you must complete the following acknowledgments:

  • Weekly acknowledgment: A detailed review of your PHI responsibilities, including third-party AI processing disclosure. Valid for one calendar week (ISO week). Required before your first session each week.
  • Daily acknowledgment: A brief reminder confirming you understand the requirement to keep patient identifiers out of your dictation. Required before your first session each day.

Both acknowledgments are timestamped and logged for compliance documentation. Failure to complete the required acknowledgments will block access to dictation and note generation features until completed.

Identifier-detection gate

EchoMed scans your dictation input for 12 patterns of potential patient identifiers. If a pattern is detected, a blocking modal requires explicit acknowledgment before processing continues. Overrides ("Generate anyway") are recorded in the audit log. Generated output is also scanned and flagged for review.

Review confirmation before export

After a note is generated, copy, download, and share actions are disabled until you confirm you have reviewed the note for accuracy. This is a deliberate gate intended to keep the clinician in the loop on every note that leaves the system.

Acceptable use

Do not upload content you are not authorized to process. Do not use the service for emergency or life-critical decisions.
