How It Works
Turn raw visit details or dictation into structured clinical notes in a few steps. Built by a practicing clinician who documents the same visits you do.
Select reason for visit
Choose from 13 standard visit types: SOC, skilled nursing, wound care, catheter care, recertification, discharge, and more.
Review guidance and format
A short prompt appears to guide dictation. Choose DARP, SOAP, or Narrative if you want to override the default.
Add optional guidance
Use custom templates or preserve terms to match agency requirements and protect product names.
Dictate or paste
Record a summary or paste your visit details. Dictation appends to the summary field.
Generate and review
Click Generate Note, then review for accuracy and edit as needed.
Copy into your EHR
Copy to your EHR or email a formatted note. Usage tracked automatically.

Real dictation is messy. Here’s the kind of rough, spoken input EchoMed takes — and the structured DARP note it returns.

Structured DARP note
DARP, SOAP, and Narrative formats are available, plus custom templates. This is an illustrative example — keep real patient identifiers out of dictation.
Tips for best results
- Say brand names and dressing types clearly
- Include vitals and wound measurements
- Do not include patient identifiers
